We invite
you to come and visit our facility with your loved one. Our staff will be happy to meet with you and show you all the fun
events we enjoy at Elderday Center. Call 630-761-9750 to schedule a tour. After you've scheduled a tour, one of our social workers will schedule an assessment
with your loved one and any primary caregivers. This allows both parties to determine how to best serve the older adult. The
assessment is paid for by Medicare. Call 630-761-9750 to schedule.

must come in its original container), monitor blood pressure and weight, serve as a liaison between necessary parties and
provide practical insights into the physical aspect of caregiving. Does my loved one need to come every day? No. Minimum program attendance are for two days a week for a total of 10 hours (typically recommended 5 hours

get the maximum benefit out of our program. How
