Admission Requirements:  Over 55 years old Continent of bowel and bladder (ability to toliet self) Ability to feed self Ability to move on own or with a walker, wheelchair or cane Free of communicable disease Ability to participate in group activities

ADMISSIONS PROCEDURES We invite you to come and visit our facility with your loved one. Our staff will be happy to meet with you and show you all the fun events we enjoy at Elderday Center. Call 630-761-9750 to schedule a tour. After you've scheduled a tour, one of our social workers will schedule a 90 minute assessment with your loved one and any primary caregivers. This allows both parties to determine how to best serve the older adult. The assessment is paid for by Medicare. Call 630-761-9750 to schedule an assessment.
